North Lanarkshire Phase 2

North Lanarkshire Phase 2 provides a wide rangeof help and support for people with physical and learning disabilities including complex conditions such as:

  • mental health - dual diagnosis
  • multiple sclerosis
  • autism
  • aspergers syndrome
  • epilepsy

We also support young people with various disabilities to make the transition from childrens to adult services.

We use a person centred approach which is about putting the person first. We value and listen to the opinions of all the people we support and those of their families and carers.

We support people to live in their own homes and help them maintain their own tenancy. For some young people, we support them by going into their family home and help out with a variety of tasks.

We have a diverse staff team from various backgrounds. This brings with it a variety of life skills, ideal for the support offered by the project.

We would strive to offer staff who would meet your care needs and match the personality you’re looking for.

The service is currently provided in partnership with North Lanarkshire Council. Services can be arranged through your care manager. Additionally you can approach us directly to provide a service to you if you are in receipt of either direct payments or a personalised budget.
