Overview
We provide 24-hour support to individuals and groups in their own homes. People are supported socially, personally and with all aspects of their lives.
We plan with the people we support to help them to meet their outcomes and ensure they are involved in all aspects of their service.
We work closely with the people we support, their families and other professionals to make sure that the service provided is appropriate and the best that we can give.
Providing support for six men, enabling them to live in their own homes in West Dunbartonshire.
A dedicated staff team and team leader strive to ensure that the people using the service have every opportunity to live fulfilling lives as part of their broader community. This is achieved by encouraging the individuals and their families to identify needs, preferences and aspirations. Support offered can cover a range of daily living skills including shopping, cooking, money management and personal care. We are also able to provide support with short breaks. In order to provide a holistic model of support, we use a multi-agency approach which includes strong working relationships with Health, Social Work and other organisations.
Each person has a comprehensive support plan which is reviewed regularly to ensure it is meeting the individual’s needs.
We aim to provide a high quality service which is tailored to the needs of each individual and supports the person to develop their full potential. The service is funded by West Dunbartonshire Council and there is also a personal contribution based on a financial assessment. The service is registered with SCSWIS and referrals for support can be sought through Quarriers.
